Often, being asked about not controlled forms, my clients reply: “This is “just a form” a form.” I always wonder why should a form be treated differently than any other instruction or a document? If a form is not controlled, how would we know that we need it? If it is not controlled, it cannot be referenced! If your forma are not controlled, how would you know that you use the latest revision of it? Well, exactly what is a form? A quick quiz will help answer this question. If we have a list of directions telling us to:
- draw a two-column table
- enter your company name into the first column
- enter your company?s URL into the second column
Hardly anybody will argue that this three-line direction is an instruction to make and complete this form. So if this is an instruction, it “shall” be controlled, right?
Now, let’s assume, somebody gave us a two-column table. We also were asked to fill it out. The first column is titled “You company name” and the second column “Company URL”. I bet, most of us would enter our name in the first column and our phone number in the second. Does it mean that we treated the table as an “instruction”? We did!
If we agree that our first three-line instruction in English was a ?real? instruction, that needs to be controlled, the second, completed form, resulting in the same output, must also be an instruction!
It seams to me that misunderstanding concerning forms is because forms serve two purposes. Blank forms are short directions written in tabular language. When a form is completed, it becomes a record. Not like instructions, records are controlled by different means. Let’s realize this difference and treat not completed forms as any other document controlled by our documentation management procedure. There are a couple of simple tests you may take when you are tempted to use a form that has not been assigned a part number:
- If you created a form and found it had been changed, would you like to know who did it and why?
- If you changed your form, would you like personnel to use the most resent revision?
- If you were on vacation, would you like folks to be able to find your form just by finding a reference to it?
If you answered, “yes” at least once, your form should be controlled per your documentation management procedure.
Well, formally there are a number of definitions of what a quality manual is. These definitions are in ISO 10013, ISO 9001 and other standards. In fact, a quality manual is a top-level document defining your quality management system. A quality manual establishes the policy level position of your management in the area of quality management system. There are two principal definitions of what a quality manual for an ISO 9001:2000 QMS should be:
ISO 10013, Guidelines for Developing Quality Manuals, element 4.2, gives detailed suggestions for creating a quality manual. It defines a quality manual, among other requirements, as a document that should “consist of, or refer to, the documented quality system procedures intended for planning and administration of activities which impact on quality” ISO 9001:2000, element 4.2.2 describes a quality manual as a document containing:
- the scope of the QMS and details of and justification for any exclusions;
- the documented procedures or reference to them;
- relationships between the processes of your quality management system.
Let’s follow these simple instructions and see how easy it is to create a quality manual to document the scope of your quality management system and exclusions; ensure that procedures are referenced and specify relationships or interactions between your processes.
Most companies I worked with, state the scope of QMS in a scope or introduction sections of their manuals, similar to these: “XYZ Corporation designs, develops, manufactures and sells industrial application flow meters” or “XYZ Corporation provides paper and plastic recycling services to municipalities in the Great Newtown area”
When the scope is defined and exclusions are determined, it?s time to describe used processes and determine reference to the corresponding procedures. As I found through my consulting and auditing work, the best way to start this step is to transform the standard from a set of applicable requirements into your company?s commitment to satisfy those requirements. For example: If the standard in element 5.6.1, requires that the “Top management shall review the organization?s quality management system at planned intervals, to ensure its continuing suitability, adequacy and effectiveness,” our manual will state: “My Company, Inc.?s top management review the quality management system on at least a quarterly basis to ensure its continuing suitability, adequacy and effectiveness per the Management Review Procedure.”
As you can see, a reference to a specific procedure satisfies the second requirement for a quality manual. Following this approach by addressing all applicable elements of the standard and referencing corresponding procedures, we will establish a manual that meets the requirements of ISO 9001:2000 standard.
Various methods are used to document process interactions. Some companies utilize Figure 1 in ISO 9001:2000 standard to document relations between main processes.
